Office Policies at Our Thousand Oaks Podiatry Practice
At Performance Foot & Ankle, we want every visit to be smooth, efficient, and focused on your care. The policies below explain how appointments, cancellations, urgent concerns, and first visits work at our office so you know exactly what to expect before you arrive.
Request an AppointmentAppointments
Our office operates by appointment only. Please call (805) 273-1415 ahead of time to schedule your visit. We do our best to accommodate walk-ins and same-day appointments whenever possible, especially for patients experiencing new or worsening pain.
Cancellations and Rescheduling
If you need to cancel or reschedule, we kindly ask for at least 24 hours’ notice. Advance notice allows us to offer your appointment time to another patient who needs care. Thank you for your understanding.
Urgent Foot and Ankle Concerns
We understand that injuries and urgent problems can happen at any time. We strive to be available when our patients need us most and provide in-office digital x-rays as needed, so many injuries can be evaluated right away. If you suspect a fracture, notice signs of a foot infection, or have a new wound and diabetes, call our office as soon as possible. For life-threatening emergencies, call 911 or go to the nearest emergency room.
Your Initial Consultation
For your first visit, we recommend arriving 30 minutes before your scheduled appointment. This gives you time to complete any necessary paperwork and helps us keep your visit on schedule. During this time, our team will review your medical history and current health status so your podiatrist has a complete picture of your needs and concerns.
To save time, you can complete your paperwork in advance on our online forms page. Please bring:
- Your insurance card and a photo ID
- A list of current medications and allergies
- Any recent x-rays, MRIs, or records related to your foot or ankle condition
- The shoes you wear most often, if your concern involves pain while walking or exercising
Insurance and Billing
We accept a variety of PPO plans and Medicare. We do not accept HMO plans. We recommend checking your benefits before your visit, and our staff is always happy to help. Visit our insurances we accept page for a list of plans.
